Which endometrial test, and does it matter?

A companion to the chronic endometritis episode. Compare CD138, hysteroscopy, culture, EMMA/ALICE and vaginal microbiome testing — and see when investigating is worth discussing.

Limits
These tests are not interchangeable. A positive on one doesn't mean the others will agree — and a finding by itself doesn't prove treatment will improve your live-birth chances.
Repeated failed embryo transfers (recurrent implantation failure)
Two or more pregnancy losses (recurrent pregnancy loss)
Persistent abnormal bleeding or spotting between periods
A hysteroscopy showed inflammation or other lining findings
A first transfer failed with a good-quality embryo and nothing else explains it yet
Educational only. Antibiotics should only be started on your doctor's advice. Per the episode, ASRM does not support routine antibiotic use before embryo transfer, and finding bacterial DNA does not automatically mean treatment will raise live-birth rates.
